The Ontario Federation for Cerebral Policy (“OFCP”, “we”, “us”, “our”) knows that your privacy is important to you and we are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the services we provide. We will make every effort to ensure that any personal information that we obtain will be kept private and confidential.

Personal information means any information that may be used to identify an individual, including, but not limited to, a first and last name, a physical address and personal situations.

When you use our website or services, we may collect the following personal information from you:

• Contact information such as name, email address, mailing address and phone number;
• Demographic information such as gender, age, marital status and location of residence (city/province/country);
• Health-related information such as medical and diagnostic letters from health professionals confirming cerebral palsy diagnosis and a medical-related need for assistance and equipment; and
• Transaction-related information such as credit card number and billing address (see below for further information).
  

Our Services

At the OFCP, our principle activities involve providing support services (“Services”) to people with cerebral palsy of all ages in the province of Ontario, their families and professionals. While these activities may change from time to time, our principle activities include:

• Providing information referrals, including to support services, parent support services, housing services and employment services;
• Operating funding programs to assist with purchasing equipment and assistance-related devices;
• Organizing emergency relief assistance; and
• Providing financial assistance to assist our members who have cerebral palsy.

Donations 

We also collect personal information in order to process donations. We raise funds through online donations via an agreement with the organization CanadaHelps, a registered charity. You can visit CanadaHelps’ website here: https://www.canadahelps.org/en/.

While processing donations on our behalf, CanadaHelps will collect your personal information to process the donation, including contact information and banking information. We note that CanadaHelps may process your personal information outside of Canada, including the United States. You always have the option of remaining anonymous, in which case CanadaHelps will not share your personal information with us. Otherwise, we have the ability to see your contact information and we may contact you to follow up with your generosity.

If you choose to donate to us via regular mail, the phone or in-person, we will internally collect and use that information to process your donation.

Contacting You

Our website includes forms where you can voluntarily provide us with personal contact information so that we may follow up with you.

Third Parties

We will not sell or distribute your personal information to third parties, except with your express consent as outlined in this Policy.

CanadaHelps: As described above, we process all online donations through CanadaHelps, a registered charity.

Service Providers: We may share the personal information we collect from you with companies or individuals that provide services or perform functions that enable us to serve you. At the time of collection, we will identify the specific service providers with whom we may share your personal information, and ask for your express consent to do so. We will only share a limited amount of personal information with our service providers, as is required for processing purposes. We require all our contractors to agree to protect personal information using safeguards reasonably similar to ours. Further, we require our service providers to restrict any use of such personal information to the purposes for which we have engaged the service provider.

Government and Legal Purposes: We reserve the right to disclose your personal information in special cases, such as when required by law, to comply with a judicial proceeding, court order, or legal process served on us, to enforce or apply applicable terms and conditions and other agreements, or to protect the rights, property, or safety of our organization, the Community, or others.

Cookies: Our website (www.ofcp.ca) collects non-personal information such as your network location (IP address), the pages you visit, time of visit, your operating system and browser, in order to generate website traffic reports only.

How We Secure Personal Information

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, technological and organizational measures to safeguard and secure the information we collect. We store all personal health information that we collect on internal servers located in Ontario. 

Retention

We keep personal information only for as long as required to fulfil the purposes for which it was collected. However, in some circumstances, we may retain personal information for other periods of time, for instance where we are required to do so in accordance with legal, tax, and accounting requirements, or if required to do so by a legal process, legal authority, or other governmental entity having authority to make the request, for so long as required. In specific circumstances, we may also retain your personal information for longer periods of time corresponding to a statute of limitation, so that we have an accurate record of your dealings with us in the event of any complaints or challenges. When personal information is no longer necessary for such purposes, we will take steps to have it deleted, destroyed, erased, aggregated, or made anonymous.

Individual access

You may request access to your personal information held by us. To protect your privacy and security, we will take reasonable steps to verify your identity before granting access.

Changes to this Privacy Policy

We reserve the right to update this Privacy Policy from time to time for any reason. We will notify you of a material change to this Privacy Policy by placing a notice on or website, or by notifying you via email or postal mail.

Challenging compliance

If you wish to make a formal complaint about our privacy practices, you may make it in writing to our Information Officer. We are committed to resolving disputes in a timely manner. We will investigate your complaint and respond via email, fax or telephone.
