What the Program Does?

The Long Term Planning and Support Program (LTPSP) assists families and people with CP in planning for the future by providing a wide range of resources and services. One of the most unique and compelling features of the program is that it provides Lifetime Supports Services to the person with CP when their parents have passed away or when they are no longer able to continue to provide support.

The assistance provided is customized to the needs of each family and can include:

  • Home Visits
  • Future Care Planning as well as Emergency Planning
  • Creation of Medical and Emergency Data Packages including ID Card and Emergency Booklet
  • Creation of “Circle of Friends” to keep people connected
  • Estate Planning information sharing regarding Wills, Henson Trusts, RDSP’s and Powers of Attorney
  • Responding to Urgent Needs

Who the program is for?

The Long Term Planning and Support Program welcomes any family with a member who has Cerebral Palsy. The family member with CP can be of any age and must live in Ontario. It is also available to any person with CP who wishes to join on his/her own behalf.
